IMPORTANT ANNOUNCEMENT! Physicians and CCVS Customers
Effective June 1, 2017, the Arkansas State Medical Board will roll out the first phase of their CCVS ordering changes. These changes have been discussed at various meetings over the past year and the time has come to make them effective for the benefit of our customers, physicians and the ASMB/CCVS. Prior to placing an order for a CCVS profile, the customer organization must provide the following items, if they are not current or on file with the CCVS:

  • Organization Specific Authorization and Release – the Organization name must be entered exactly as it is listed on the CCVS customer’s current account. Abbreviations (unless listed on their account), altered forms and blank lines will not be accepted. The A&R must be current within the two-year time from the signature date. Organization-specific A&Rs can be found on the CCVS website at www.arccvs.org.
  • Current ASMB Authorization and Release – This is the Board A&R the physician is supposed to sign once a year at license renewal. It must be current within the two-year time frame with no alterations to the form. It can be found on the ASMB web site at www.armedicalboard.org.
  • Current CCVS Attestation – Signature date must be within 120 days. Form can be found online or at www.arccvs.org.
  • Current Curriculum Vitae (CV) – Must be dated within 120 days. Active Arkansas License – Please check the license status prior to ordering a CCVS profile. This verification is free at www.armedicalboard.org. If you need assistance, please contact us.
In addition, the ASMB/CCVS new payment option of credit card only will be implemented on June 1, 2017.
